When this adjustment is made should the winding click [ratchet] be pulled all the way with the smaller spring. Or should it be in the middle or start. When installing the motor the bellows make the chain go somewhat slack.

I don't fully understand what you are trying to say, but the chain should be right next to the pulley and there should be no slack what-so-ever on the chain once the bellows are istatlled. If there is slack the the pin dislogues and the chain will be pulled away from the coil spring and into the frame. My guess is that the smaller spring which is attached to the winding arbor is spent or too long. Try shorting or replace.
