Is the lever inside the plates? If so, it is probably the chime correction lever.. brings the chime and hands back in agreement when they get off. Clock will work without it, but will not self-correct.

Misallignment of the two "cams" on the front of the plate on the chime train is the problem. Or possibly too little warning in the chime train

Not sure I can describe how to set it up properly without one in front of me. Perhaps someone else can do it from memory.

This is a good learning opportunity as most chiming clocks use some variation of this set-up to do chime correction.

It is indeed the chime correction lever. Our experience generally is that does not lift high enough on the hour.

There is a simple fix. The chime unlocking lever is a v-shaped device actived by the star wheel on the minute arbor and raises the locking lever for the chimes. Open the V somewhat and it will raise the locking lever higher.

Mark has described the problem perfectly. The cause of this is the customer spins the minute hand forward without waiting for the quarterly chimes. Jamming occurs and the v shaped lever squeezes a bit and now cannot lift high enough for the self correcting device.
