18 size, 11 jewel, model 5, Hunting, Grade 99 is what the Megger's book states for this serial number.

We can't tell your location... but there may be a local watchmaker that can help you with the case screw, the bow is one of those things that should be seen to match the case proportions...and be fitted to the case.
Polishing.... It is your case, there are differing opinions on preserving the patina of silver cases.....

On polishing the case or not? Do what you like. If the case is free of dings and nicks they look good with the light polish you will get from carrying it. If you plan to display the watch I like them polished shiney as long as the rest of the watch looks that good also. So, it is just a matter of your preference. Remember, it took a long time for the case to tarnish or "patena" like it is and only takes a minute to wipe it off with some polish. so make sure what you want before you polish. As far as parts for the case and such go, get on the hunt. Check with members of your local NAWCC chapter and go to the meetings. There is sometimes a mart @ the meetings. You can find these parts and probably not too hard either. I have had to buy a case for a bow or crown but sometimes thats how it is. I prefer to use old parts in good shape rather than new. I think they look better and you can always tell a repro piece. As far as a case screw goes find a watchmaker in the area and get to know him if it is worth it. I have good luck comeing up with these type of parts like that . Well I hope this helps and good luck.
